2016 Graduation Info

We are gathering information regarding graduates in our church family in order to honor them during a Sunday morning service in June.  Please complete the following information if you are graduating from high school, college, trade school, university or any other institution (or you know someone in our body who is) in the 2015-2016 school year.

Please note that the below fields marked with a red star (*) are required.  We ask for contact information of both the person submitting the information and the graduate so that we can clarify information if needed.
